Replacement of Lost Results or Receipt Order Form

If you have unfortunately lost or misplaced your Test Recording Form or your Test Receipt, please complete and submit.

It has been made very clear that it is the test candidates responsibility to forward copies of their test results to all fire bases that they are applying as well as updating their on-line application information. Your Fire Manager requires the original copy of your results on your first day of employment, a photocopy is not acceptable. A $25.00 fee is required per document, paid in advance, to either verify your results with your employer or to re-issue documents. Anticipate a minimum of 3-5 days for documents during the testing season. They will be forwarded by fax to your Fire Centre clerk/manager. Documents will not be sent electronically.
